*Summary*

We are seeking a dedicated Physical Therapist to join our team at Eagle Rock Health and Rehabilitation of Cascadia. In this role, you will provide exceptional rehabilitation services to our patients, focusing on their individual needs and goals to promote recovery and improve their quality of life.

*Responsibilities*
* Develop and implement individualized treatment plans based on patient assessments.
* Provide hands-on physical therapy interventions to enhance mobility and function.
* Monitor patient progress and adjust treatment plans as necessary.
* Collaborate with interdisciplinary teams to ensure comprehensive patient care.
* Educate patients and their families on treatment goals and home exercise programs.
* Participate in discharge planning to facilitate smooth transitions for patients.

*Requirements*
* Licensed Physical Therapist in the state of practice.
* Strong background in patient care with a focus on rehabilitation.
* Knowledge of medical terminology related to physical therapy practices.
* Experience in geriatrics or pediatrics is preferred but not required.
* Excellent communication skills for effective collaboration with patients and healthcare teams.

Job Types: Part-time, PRN

Pay: $42.00 - $48.00 per hour

Expected hours: 10 – 20 per week
